PACKAGE FOR THE SIERRA MODEL
There are two Models of service delivery to our independent and assisted living communities:

Model I:
Sierra Therapy Services establishes a fully equipped, freestanding clinic within the retirement community in a designated leased space. [The Assisted Living Community might need a waiver to allow leasing of otherwise designated space from the State at a Certificate of Need level] The Independent Living Community does not require a waiver.

In exchange for rent, Sierra Therapy Services’ personnel carries out the following activities:
  • Visits to identified hospitals and other rehab institutions to assist in the assessment process of potential residents and return residents.
  • Presentations to the community, hospital discharge planners and nursing homes to develop continuum of care for patients – both short and long term – with LESSOR as an integral part.
  • Develop appropriate brochures and flyers for LESSOR as needed that reflect its programs and excellent facility for the retired elderly.
  • Education and training – provide facility, family and caregiver training which focuses on the integrated team approach to care and wellness of the residents at LESSOR. This staff education program is based on the Assisted Living Federation of America’s (ALFA) Training Series and Sierra Therapy Services’ Education Program.
  • Assist with the development of appropriate wellness programs.
  • Develop a marketing plan in collaboration with members of LESSOR management team for the promotion of this facility, highlighting the excellent programs and value.
  • Regular planning meetings to target and develop future programs for LESSOR will be led by identified LESSOR Personnel.
Model II:
Sierra Therapy Services does not establish a clinic space but provides out patient services to the residents of the community. As Medicare Part B Assigned Providers, services will be covered when provided at the therapist’s office or the patient’s home. Sierra Therapy Services will be very timely in responses to referrals to provide services under the Sierra Model.
